Kohima, April 19 (DIPR): The Department of Health & Family Welfare invites application for local intending female candidates for undergoing training in Female Health Workers (FHW) for the Session 2011- 2012.
(a) Female Health Workers one and half years course at District Hospital Tuensang.
(1) Only unmarried candidate may apply.
(2) Original Educational Qualification/Documents should be produced before the Interview Board as and when called for. Photostat copies duly attested by Gazette officers may be submitted. Indigenous certificate is compulsory for all (Photostat copy).
(b)Required Educational Qualification :-
(1) HSLC or equivalent from any recognized Board of examination.
(2) Candidate should not be less than 15 years and not more than 35 years as on 3l/12/2012.

- Issue of application form- April 27 onwards.
- Last date of form submission- May 20 2011.
- Issue of Admit Card - June 8 onwards.
- Date of Written Test- June 25, 2011.
Incomplete documents/form will be rejected. No separate calling letter will be issued.
Venue of interview will be given in the Admit Card Forms will be available during office hours at the Directorate of Health & Family Welfare, Secretariat Road, Section-5 (Training) Room No-210 at the cost of Rs 100 only.
